DOVER — Following an investigation into the death of a man who froze outside a veterans home in February, the Wisconsin Department of Health Services has cited the Wisconsin Veterans Home for two state violations.
Norman Wagner, 87, died of hypothermia Feb. 3, after he walked outside his home at Gates Hall on the Wisconsin Veterans Home campus and could not get back inside.
At the time of his death he was living in an assisted living apartment complex at the veterans home on Walnut Drive. The veterans home was cited for not providing sufficient services and not providing an adequate risk agreement for Wagner to sign, according to Steve Gloe, an attorney for the Department of Health Services.
A risk agreement is a document which residents sign that identifies their preferences for care and risks associated with their preferences. There was a $1,000 fine associated with the risk agreement violation, Gloe said.
Because the report has not been finalized, Gloe said he could not elaborate on what services the veterans home did not provide.
As a part of the appeals process, veterans home officials have agreed to install emergency alarms on the doors and have staff undergo additional training, Gloe said.
The Wisconsin Department of Veterans Affairs is also doing an investigation, said Andy Schuster, the department spokesman. But because that investigation is ongoing, Schuster said he could not comment on it. Officials at the Wisconsin Veterans Home did not return calls to comment.
