RACINE COUNTY - People who are planning to attend the H1N1 vaccination clinic Saturday at Case High School are asked to fill out a consent form ahead of time to help expedite the process. The clinic is 9 a.m. to 3 p.m. Saturday. The form is available online here.

The form is also available at the Racine Public Library, 75 Seventh St. in Racine; the Caledonia/Mount Pleasant Health Department, 10005 Northwestern Ave. Suite A in Franksville; the Western Racine County Health Department, 156 East State St. in Burlington; and the City of Racine Health Department, 730 Washington Ave. in Racine.
